Overview

Palliative care is a people-centred holistic approach to enhancing the quality-of-life of individuals suffering from severe illnesses while lowering the socioeconomic cost imposed by the disease. The goal is to enhance the quality-of-life for both the patient and their family using a comprehensive strategy, i.e., address the physical, emotional, social, psychological, and spiritual needs of the patient while providing relief from myriad disease-associated symptoms, like pain, loss of appetite, nausea (urge to vomit), shortness of breath, and fatigue. Palliative care can be employed for patients at all stages of life and at any stage of a serious illness, and can be integrated with treatments aimed at curing disease. It is generally used for patients affected by conditions, such as cancer, end-stage disease, severe burns or trauma, organ failure, diabetes, chronic obstructive pulmonary disease (COPD), rheumatoid arthritis, or neurodegenerative diseases, like dementia, Alzheimer’s disease, Parkinson’s disease, or ALS (amyotrophic lateral sclerosis). The range of locations at which palliative care can be provided is immense, i.e., in hospitals, outpatient clinics, nursing homes, and even homes.

The Department of Pain and Palliative Care at Sir H.N. Reliance Foundation Hospital functions with a mission that no patient should ever be denied any surgery/procedure to alleviate their suffering, and patients—and their families—with serious illnesses should be provided symptom relief, dignity and support at all times during their treatment journey. The Palliative Care and Pain Management Department at Sir H.N. Reliance Foundation Hospital, which hosts state-of-the-art equipment and provides cutting-edge services, aims to provide compassionate, comprehensive care to patients with serious illnesses, ensuring comfort, dignity, and the highest quality-of-life possible.

It is led by Dr Mahesh Menon who has dual expertise, i.e., in both chronic pain medicine and palliative medicine, which positions Sir H.N. Reliance Foundation Hospital at the top of the corporate medical sector, offering comprehensive care to all people and families who approach it for medical help.

In India, less than 2% of individuals with advanced cancer have access to palliative care. The Palliative Care and Pain Management Department at Sir H.N. Reliance Foundation Hospital caters to individuals who approach the various departments at Sir H.N. Reliance Foundation Hospital. Our department plays a key role in providing support to terminally ill patients and their families, and aims to alleviate patient suffering and help them understand the treatment options in an attempt to improve their quality-of-life.

The Tower

The Pain and Palliative Care Department at the H.N. Reliance Foundation Hospital provides a variety of services; the same have been enumerated below.

  • Diagnostic tests and procedures:
    • C-arm guided diagnostic nerve blocks for chronic pain
    • Point-of-care ultrasound-guided diagnostic nerve blocks for chronic pain
  • Treatments for pain:
    • Pain nerve blocking injections or nerve blocks:
      1. For people with chronic spinal pain, back and neck pain and joint pain
      2. For people with illnesses like cancer, neurological problems, kidney or liver failure
      3. For people with sports injuries
    • Pain nerve ablations (or radiofrequency ablations to eliminate pain-causing nerves using heat energy):
      1. For spinal pain
      2. For conditions like trigeminal neuralgia (TGN) that cause facial pain
      3. For conditions like headache
      4. For conditions like knee, shoulder, or hip pain

    • Pain interventions and minimally invasive pain and spine interventions (procedures in which small cuts and specialised tools are employed to access the spine and treat pain and spinal conditions):

      5. Ganglion impar injection
      6. Glossopharyngeal nerve block
      7. Intercostal nerve block
      8. Radiofrequency ablation for spinal pains
      9. Radiofrequency thermocoagulation trigeminal ganglion
      10. Regenerative medicine including platelet-rich plasma injections
      11. Sphenopalatine block
      12. Splanchnic nerve block
      13. Stellate ganglion block

    • Treatment strategies in palliative care:Treatment strategies are highly personalised and involve offering multi-disciplinary meetings, analysing case histories in detail, and tailoring a plan involving:
      • Medical management using the best possible treatment guidelines
      • Minimally invasive surgical procedures and pain management interventions
      • Supportive management including psychosocial support and rehabilitation services
      • Establishing home care plans
