Patient Rights

At Sir H. N. Reliance Foundation Hospital, we prioritise your healthcare rights and ensure that every patient receives the highest level of care. As a patient, you have the right:

  • To receive care in a safe, clean environment, free from discrimination based on age, gender, race, religion, nationality, sexual orientation, disability, or mode of payment.
  • To receive considerate and respectful care that supports personal dignity and protects against abuse, neglect, or harassment.
  • To be respected for special preferences, personal values and beliefs, spiritual and cultural needs.
  • To seek a second medical opinion for your desired diagnosis and treatment.
  • To expect confidentiality of your medical information, ensuring that patient privacy and medical confidentiality are strictly maintained, and to provide personal dignity during examination, procedures, and treatment.
  • To control what personal health information is shared with family/others and under what circumstances.
  • To receive appropriate assessment and effective management of pain pertinent to the medical condition.
  • To receive dignified and compassionate care at the end of life.
  • To be informed by your doctor regarding the proposed treatment’s risks, benefits, alternatives, and expected outcomes.
  • To knowledgeably refuse any care, diagnostic procedure, and treatment after the consequences of refusal have been explained.
  • To give informed consent before undergoing surgeries, anaesthesia, blood transfusions, initiation of any research protocols, or other invasive/high-risk procedure(s)/treatment.
  • To be informed of the process for voicing complaints to express concerns, violation of your rights and/or grievances.
  • To access your medical records and stay informed about your care plan and treatment progress.
  • To know the identity and qualifications of your healthcare providers.
  • To be fully informed about the financial aspects of your treatment, including the expected cost of care.
  • To understand and comply with the hospital’s rules, regulations, and patient safety guidelines.

Patient Responsibilities

As a patient, you also have responsibilities to ensure the effectiveness of your treatment and the smooth functioning of the hospital:

  • To provide correct and complete personal information, including full name, age, address, telephone number, email ID, particulars of next of kin, and insurance company/TPA/employer information.
  • To share comprehensive details about your health history, including past medical conditions, medications, and treatments.
  • To actively participate in your healthcare, following the recommended treatment plan and updating your care team about any changes in your condition and the effectiveness of your treatment.
  • To attend scheduled appointments and inform the hospital if rescheduling or cancellation of any appointment is required.
  • To respect the rights of other patients, visitors, and hospital staff and treat them with courtesy and dignity.
  • To comply with hospital care guidelines, including maintaining low noise levels, adhering to visitor policies, and following hospital rules on prohibition of carrying weapons, smoking, tobacco, and alcohol use.
  • To avoid bringing valuables to the hospital and assume equal responsibility for the safety of personal belongings.
  • To provide complete and accurate information to the insurance company to process insurance claims.
  • To assure that the financial obligations of one’s health care are fulfilled as promptly as possible.
  • To respect the fact that patients requiring emergency care can take priority.
  • To be respectful of the property of other individuals and the hospital.
  • To respect the hospital's protocols for patient safety, privacy, and the confidentiality of medical records.
  • To communicate with the healthcare staff if the condition worsens or does not follow the expected course of treatment.
  • To carefully understand instructions before signing any consent forms for procedures or treatments.
  • To understand the charter of ‘Patient Rights’ and seek clarifications, if any.

By understanding your healthcare rights and responsibilities, you play an active role in ensuring that you receive the best possible care while supporting the hospital’s commitment to patient care ethics, safety, and high treatment standards.
