Customers will not be able to unsubscribe from these notices.
  2. Information shared: We are the sole owners of the information collected on this website. We will not sell, rent, or otherwise disclose your personal information to any third party, without your consent except under the following circumstances:

Introduction

  1. This serves as a notice describing how personal information about you ("you" or "your") may be collected and dealt with by Sir H. N. Reliance Foundation Hospital and Research Centre (Sir H. N. Reliance Foundation Hospital) ("Hospital" or "we" or "us" or "our") and stipulates our governing privacy policy in that regard.
  2. You acknowledge that you have read, understood and have agreed with this policy. You agree that on this basis, by installing the Application you have voluntarily exercised the option to provide to us your personal information, consented to it being governed by the privacy policy, and you are notifying us of your election and consent. You agree at our request to confirm this in writing by email, fax or letter.
  3. You subsequently have the option to withdraw your consent to provide personal information but you will have to do so specifically by clicking the relevant boxes on your screen and in writing, and following the withdrawal we may discontinue for you the purpose for which the personal information was sought.

Personal information and collection

  1. For the purpose of this policy, only information which relates to a natural person which is capable of identifying such person shall be ‘personal information’, and other information will be outside the scope of this policy unless otherwise required prospectively under any applicable Indian law. Personal information may include name, address, contact numbers, e-mail address, date of birth, medical records and history, physical, psychological and mental health condition, biometric information, other personal and health related information.
  2. We may collect personal information from you by your feeding it through the Application software / by clicking the relevant boxes on your screen, e-mails, fax, or any other form of written communication, which you may elect to use for this purpose.
  3. Personal information sent by your web browser may be automatically collected. This information typically includes your domain name (the site after the ‘@’ in your e-mail address). It may also contain your user name (the name before the ‘@’ in your e-mail address). Other examples of information collected by our server include the internet protocol address used to connect the user’s computer or mobile to the internet, operating system and platform, the average time spent on the Application, pages viewed, information searched for, access times, applications visited before and after a user visits the Application, and other relevant statistics. The amount of information sent depends on the settings on your browser on the Application; please refer to your browser to learn the information that it sends.

Purpose and use

    1. to enable you to download the Application, product information, service information, order products and services related details and make use of the Application.
    2. to enable us to provide information on products and/or services through the Application, to your e-mail address or, by post, to your postal address or otherwise.
    3. to create an individual profile for you so that we can understand your requirements, health related data provided by you and inform you of our or third party products, services or special offers that may be of interest to you.
    4. to process appointments with doctors or health check-up services applications submitted by you and to set up or remind you about future appointments.
    5. to provide, coordinate and manage your care. This will include communication and consultation among healthcare providers, and other members of the medical team, and disclosures for treatment purposes, both within and outside of the Hospital.
    6. for payment purposes – creation of bills and collecting payment from you or insurance companies, medi-care etc., and this may entail use of information such as dates of services, symptoms and diagnosis.
    7. to seek your feedback or to contact you in relation to the product and/or services offered on the Application.
    8. to your family or friends involved with your care, as we find patients often discuss their care with such persons.
    9. to improve the quality of care we provide to patients or to run the Hospital and health care facilities. These include activities to monitor and improve patient care, enable staff to care for patients, prepare for State and Central regulatory reviews, train health care and non-health care professionals, manage health care operations and improve health care services.
    10. for medical or scientific research.
    11. for maintaining a statistical database.
    12. to seek tax-deductible contributions to sustain our healthcare mission, and for this we may use only your basic information and the dates on which you were at or treated by the Hospital.
    13. to administer or otherwise carry out our obligations in relation to any agreement or arrangement you have with us.
    14. to anticipate and suggest possible resolutions in respect of any products or services sought to be availed by you.
    15. to process and respond to requests, improve our operations, and communicate with users about our or third party products and services.
    16. to enable you to subscribe to newsletters if any.
    17. to enable us to respond and provide assistance in cases of emergencies, for compliance with any law, regulation, judicial or government directive and for public health purposes.

  1. You will ensure the information (including personal information) provided to us is true, accurate, complete and up-dated from time to time in all respects, and not misleading in any manner. You will indemnify us and hold us harmless for any loss we may incur or suffer arising out of any such information being untrue, inaccurate, incomplete or not up-dated.
